  • Clinical Liaison: Full time




    Qualified RN needed to interface with referral sources providing for continuity of care between facility and home at the time of discharge for home health and hospice patients. The position requires evaluation of patient at the time of referral, identification of services required by the patient and interactions between patient, caregivers, facility and other providers to coordinate care. The position requires the ability to build relationships with referral sources, physicians and discharge planners. The ideal candidate will have strong clinical, customer service, organizational and interpersonal skills. This position is part of the clinical management team.


    Requirements:
  • PA licensed RN, BSN preferred
  • 5 years of nursing experience preferred
  • Previous homecare/hospice experience preferred
  • Demonstrated customer service ability
  • Computer skills preferred



HOSPICE POSITIONS AVAILABLE:


VNA Hospice program became the first Medicare- certified hospice to serve the terminally ill residents of Erie County.

HOME HEALTH CARE POSITIONS AVAILABLE:


RN Case Managers: Full time

VNA nurses provide one-on-one patient care with an emphasis on patient/caregiver teaching, treatments, including wound care and IV therapy; and assessments of the patient and home environment.


Our patients offer diverse medical diagnoses and health challenges that allow our staff the opportunity to have a variety of patient care experiences. Our patients frequently have conditions such as diabetes, heart and lung disease, cancer and complex wounds.


Our Full-Time Case Managers guide the patient’s care from admission to discharge, assisting the patient to access all the services offered by our agency or in the community. Through the Case Manager’s planning, the patient is provided the opportunity to remain at home and be independent in managing their health care needs.


The VNA offers flexible scheduling and workdays that can accommodate your personal or family needs. Care is provided customarily during day shift hours with a weekend rotation.

  • Per diem Home Care and Hospice RN’s: Flexible days & hours; Weekdays, evenings or weekends

    Qualifications: PA license, Bachelor’s degree in Nursing and computer skills preferred, one year’s experience required.

  • Occupational Therapists: Part time or Contractual

    To provide quality care to our Home Care patients in Erie County. Responsibilities include plan of care development and implementation, patient/caregiver education and interdisciplinary collaboration.


    Qualifications: PA license, Bachelor’s degree and one year experience required.

  • Home Care RN Case Managers: Full Time

    Erie and Corry/Union City areas.


    Qualifications: PA license, Bachelor’s degree preferred, one year experience required, computer skills preferred.

    Skilled nursing care is provided customarily during day shift hours with a weekend rotation. The VNA offers scheduling and workdays that can accommodate your personal or family needs. Flexible days and hours; weekdays, evening or weekends.


    RN Qualifications: PA license, Bachelor’s degree in Nursing; computer skills preferred; one year’s experience required.



  • Physical Therapists: Flexible days & hours; Weekdays, evenings or weekends



    To provide quality care to our Home Care patients in Erie County. Responsibilities include plan of care development and implementation, patient/caregiver education and interdisciplinary collaboration.


    Qualifications: PA license, Bachelor’s degree and one year experience required.



  • Occupational Therapists: Part Time or Contractual



    To provide quality care to our Home Care patients in Erie County. Responsibilities include plan of care development and implementation, patient/caregiver education and interdisciplinary collaboration.


    Qualifications: PA license, Bachelor’s degree and one year experience required.



  • Speech Therapists: Contractual



    To provide quality care to our Home Care patients in Erie County under a contractual agreement. Responsibilities include plan of care development and implementation, patient/caregiver education and interdisciplinary collaboration.


    Qualifications: PA license and Bachelor’s degree. One year experience and Certificate of Clinical Competency required.
