Fall Prevention
Ready•Steady•Go is an individualized, multi-disciplinary approach to the treatment of home care eligible patients who have fallen or have been identified at risk for falls. The Team is made up of Registered Nurses, Physical Therapists, and Occupational Therapists. Our team is helping patients with medication management, strengthening, endurance, balance, gait difficulties, and home modifications.
